1. Value Plan Member understands that Value Plan is not an insurance plan or policy. No payments to medical providers or members will be made by Value Plan. Actual savings may vary, depending upon location and specific services rendered. Value Plan is NOT an insurance plan but is a membership program which allows members to receive discounts on chiropractic services only.

2. Value Plan provides savings to its members on healthcare services through managed care networks. In order to access these networks and the related discounts, member or member's dependants must pay the providers promptly. Payments on all bills are due and payable at the time of service. If a member has difficulty using the program, that issue(s) needs to be addressed with the Value Plan Member and the chiropractic provider of services' office, not Soteria Healthcare Network or CareAdvantage of Arizona.

3. Neither Value Plan, nor any of its affiliated companies, nor any network accessed shall be liable for any payment to a provider accessed under the Value Plan program, or any refusal of participating providers to accept the network discounts offered under this program. Value Plan, its affiliated companies or any network accessed is not an insurer, guarantor or underwriter of the responsibility or liability of Member for Member's or Members’ dependant's chiropractice care or any other goods or services provided to Member or Member's dependents.

4. A limited directory of participating providers is provided to member upon enrollment. The providers listed in this directory are subject to change without notice. Member should verify with the doctors office their current participation as a Soteria Healthcare/CareAdvantage of Arizona Value Plan provider
before services are rendered or call Value Plan's hotline at (770) 455-8190 ext. 131 for current provider information.

5. Participating Chiropractic Providers are independent of Value Plan, its affiliated Companies and its contracted networks are not responsible for health care provided or the omission of the provision of health care by any provider. Value Plan and it's affiliated companies do not practice medicine or in any manner interfere with or participate in the provider-patient relationship. All health care decisions are between the patient and a provider. The selection of a provider is the obligation and decision of the patient and is not based upon the credentialing or any recommendation by Value Plan, its affiliated companies or contracted networks.

6. Participating Providers may be opt-out/decline to participate in Value Plan if they submit their request in writing to: Value Plan Opt-Out, 3440 Preston Ridge Road, Suite 130, Alpharetta, Georgia 30005 or fax their request to: (770) 455-4120.

7. Payments for the Value Plan program by consumers are due in advance before any ID Cards and provider directories will be sent.

8. Verbal notice does not constitute cancellation.

Returned checks, insufficient fund notices on bank drafts or denial by member's credit card company for payment of the annual program fee is deemed evidence of non-payment by a member and may cause termination of membership at the discretion of the Accounting Department. Value Plan Members failing to fulfill their financial obligation to Value Plan may be subject to attorney's fees and reasonable costs of collection.

10. Value Plan reserves the right to remove any member or provider for failure to pay or cooperate with the Value Plan program discount guidelines and the terms provided here.

11. Members may cancel their Value Plan program at any time upon written notice to the company and return of the ID cards. Cards should be mailed to: Value Plan, 3440 Preston Ridge Road, Suite 130, Alpharetta, Georgia 30005. Program fees on enrollments cancelled after the first 30 days following the enrollment date are not eligible for refund. Failure to use your discount cards does not constitute the need for a refund.

12. Value Plan and its affiliated companies fully-credential providers but cannot warrant or guarantee their performance. We invite and encourage your suggestions on ways our programs can be increasingly beneficial to our members.

Value Plan provides savings to its members on healthcare services through managed care networks. Neither Value Plan, nor any of its affiliated companies, nor any network accessed shall be liable for any payment to a provider accessed under the Value Plan program, or any refusal of participating providers to accept the network discounts offered under this program. Value Plan, its affiliated companies or any network accessed is not an insurer, guarantor or underwriter of the responsibility or liability of Member for Member's or Members’ dependant's chiropractice care or any other goods or services provided to Member or Member's dependents. 4. A limited directory of participating providers is provided to member upon enrollment. The providers listed in this directory are subject to change without notice. Member should verify with the doctors office their current participation as a Soteria Healthcare/CareAdvantage of Arizona Value Plan provider before services are rendered or call Value Plan's hotline at (770) 455-8190 ext. 131 for current provider information. 5. Participating Chiropractic Providers are independent of Value Plan, its affiliated Companies and its contracted networks are not responsible for health care provided or the omission of the provision of health care by any provider. Value Plan and it's affiliated companies do not practice medicine or in any manner interfere with or participate in the provider-patient relationship. All health care decisions are between the patient and a provider. The selection of a provider is the obligation and decision of the patient and is not based upon the credentialing or any recommendation by Value Plan, its affiliated companies or contracted networks. 6. Payments for the Value Plan program by consumers are due in advance before any ID Cards and provider directories will be sent. 8. Verbal notice does not constitute cancellation. 9. Returned checks, insufficient fund notices on bank drafts or denial by member's credit card company for payment of the annual program fee is deemed evidence of non-payment by a member and may cause termination of membership at the discretion of the Accounting Department. Value Plan Members failing to fulfill their financial obligation to Value Plan may be subject to attorney's fees and reasonable costs of collection. 10. Value Plan reserves the right to remove any member or provider for failure to pay or cooperate with the Value Plan program discount guidelines and the terms provided here. 11. Members may cancel their Value Plan program at any time upon written notice to the company and return of the ID cards. Program fees on enrollments cancelled after the first 30 days following the enrollment date are not eligible for refund. Failure to use your discount cards does not constitute the need for a refund. 12. Value Plan and its affiliated companies fully-credential providers but cannot warrant or guarantee their performance. 	Value Plan Members save 25% to 35% at all participating Value Plan chiropractic provider offices. There is no limit to the number of visits and no limit to any specific type of care; visits may be for care which is necessary for medical reasons, or for preventative or maintenance.
