Value
Plan offers any patient/consumer who would like to receive chiropractic
care with the ability to visit a participating chiropractic provider
and receive discounts on chiropractic/CAM services at a discounted
rate.
Is there
a fee to participate in Value Plan?
Answer:
No. As a fully-credentialed chiropractic provider of Soteria
Healthcare or CareAdvantage of Arizona, we are pleased to bring
this discount program to you free of charge. All we ask in return
is that you honor the negotiated discounts (outlined below), to
all valid Value Plan members when they come into your practice
with the proper ID Card. There are absolutely no claim forms to
complete when you treat a patient under the Value Plan.
The Value Plan Member is 100% responsible for payment of
the reimbursement. NOTE: The Value Plan does not replace any exisiting
benefits, is not a covered benefit and has been created solely
to provide access to discounted services off chiropractic care.
How do
I update my practice contact information to ensure that I continue
to see Value Plan patients?

Network / CareAdvantage of Arizona, is there any paperwork I need
to complete to start treating patients under the Value Plan program?
Answer:
No. If you are a fully-credentialed member of either CareAdvantage
of Arizona or Soteria Healthcare Network (Note: Soteria currently
operates in Alabama, Arkansas, Florida, Georgia, Louisiana, Maryland,
Mississippi, North Carolina, South Carolina, Tennessee or Virginia),
then you and your practice are already enrolled in and participating
providers in the Value Plan Program.
Will the chiropractor/CAM
provider have to fill out a claim form?
Answer:
No.There
are absolutely no claim forms to complete when you treat a patient
under the Value Plan. The Value Plan
Member is 100% responsible for payment of the reimbursement. NOTE:
The Value Plan does not replace any exisiting benefits, is not a covered
benefit and has been created solely to provide access to discounted
services off chiropractic care.
Does Value Plan Require Medical Referrals?
Answer:
No. Value Plan allows patients direct access to the participating
Value Plan practitioner.
Is this an
insurance plan?
Answer: No.

How
Does Offering A Discount Off My Billed Charges Ensure Me That Patients
Will Visit My Practice?
Answer: Our
marketing team is one of the best in the business. We understand the
principles of marketing and how precisely the message of your profession
should be personalized and communicated to your local
community consumer. We
market your services to current health plan clients of both Soteria
Healthcare and CareAdvantage of Arizona, potential HMO and PPO clients,
as well as directly to the consumer in areas located directly around
your practice. Additionally,
we also market the Value Plan network to small, medium and large businesses.
We also conduct mass advertising programs, design and implement direct
mail programs, list your practice on our web site (www.MyValuePlan.com)
and we also make sure that every Value Plan Member is educated appropriately
about which Value Plan provider is closest to their address.
What is required
now that I'm aParticipating Chiropractic Provider in the Value Plan
Program?
Answer:
There are no claim forms to complete. All we ask is that you honor
the discount(s) at the time of the patient's visit. NOTE: Once you
treat a patient, whether new or existing, Value Plan is designed for
those patients who have either exceeded their allowable visits or
do not have chiropractic care benefits. Patients who use the Value
Plan Card at a participating provider's practice are 100% responsible
for the payment of all services provided by a Value Plan participating
provider and are 100% responsible for any payments for cancelled of
missed appointments.
Value
Plan's participating provider network(s) and its affiliated companies
and staff are not responsible for any portion of the costs associated
with services provided through its discount program.
If
a patient already has insurance, can they still use Value Plan?
Answer:
Value Plan is designed for those patients who have either exceeded
their allowable visits or do not have chiropractic care benefits or
who may be uninsured. Patients who use the Value Plan Card at a participating
provider's practice are 100% responsible for the payment of all services
provided by a Value Plan participating provider and are 100% responsible
for any payments for cancelled of missed appointments. Value
Plan is not an insurance plan and allows customers to receive discounts
on chiropractic services only.
If
a patient does not have insurance coverage, can they still use Value
Plan?
Answer:
Yes! The Value Plan has no restrictions for usage. Value Plan
is designed for those patients who have either exceeded their allowable
visits or do not have chiropractic care benefits or who may be uninsured.
Patients who use the Value Plan Card at a participating provider's
practice are 100% responsible for the payment of all services provided
by a Value Plan participating provider and are 100% responsible for
any payments for cancelled of missed appointments. Remember,
Value Plan is not an insurance plan and allows customers to receive
discounts on chiropractic services only.
Are
Massage Therapy and Acupuncture discounts available with the Value
Plan?
Answer:
No.
Massage therapy is not a covered discount under the Value Plan program,
nor is acupuncture. In Virginia, acupuncture services are only available
from 'select' participating providers. For a list of Virginia providers
participating in Value Plan who provide acupuncture and chiropractic

How do opt-out
of the Value Plan discount program?
Answer: For
record purposes, we must have your request in writing and on your
practice letterhead if you wish to opt-out/no longer participate
in the Value Plan discount program. You may fax this request to
fax#: (770) 455-4120. All opt-out letters must have the chiropractic
provider's name, address and signature on the letter. Verbal notice
does not constitute cancellation. Value Plan and its affiliated
companies reserve the right to remove any member or provider for
failure to pay a cooperate with the Value Plan program discount
guidelines and the terms provided here.
